**Participants returning
an evaluation form at the end of their Festival stay may receive $5
of their registration fee back. Restrictions apply; details included
in registration packet received at the Festival.
Observer
/ Non-Participating Teacher
Teachers or others who wish to benefit creatively from watching class
and taking notes must register at the reduced rate. Physical participation
in classes is not permitted. Individual Festival faculty members reserve
the right to prohibit note taking in their classes. Please obtain
permission from each instructor before class.
Payment
of Fees
A minimum 50% deposit of total fees is due with your registration
form. The balance must be paid before or when you pick up your registration
materials. Payment is accepted by check, money order, Mastercard,
Visa or American Express. Please do not send cash through the mail.
Cancellation
and Refunds
All cancellations and refund requests must be made in writing. Verbal
refund requests will not be honored. If written notice of cancellation
is received on or before May 21, 2004, fees will be fully refunded.
If written notice is received on or before June 4, 2004, 50% of fees
will be refunded. After June 4, 2004, refund requests will only be
considered for documented medical reasons. Regardless of medical reason
or condition, refunds granted after June 4 will be no more than 25%
of fees paid. In all cases, the $20 registration fee is nonrefundable.
Refunds will not be issued before August 14, 2004.

Merit
Scholarships
A limited number of tuition-only scholarships are available to individuals
demonstrating a high level of commitment, technical skill, ability
and financial need. Recipients receive full or partial tuition waivers
and one complimentary ticket to Festival performances during their
stay. Housing, meals and transportation costs are the recipient’s
responsibility.
Work/Study Scholarships
A limited number of tuition-only scholarships are available to qualified
individuals, age 18 or older, willing to volunteer their time and
administrative or other skills in exchange for classes. Applicants
must commit to a two and a half week period from June 17-July 3. Recipients
receive a full two-week tuition scholarship and admission to Festival
performances in exchange for 25-30 hours per week of work. Awards
are based on financial need, relevant skills and references. Housing,
meals and transportation costs are the recipient’s responsibility.

Tuition
Waiver for Teachers
Teachers who accompany five or more students who register for the
same length of time (i.e., one week or two weeks) may waive the tuition
portion of their fees; restrictions apply. Tuition waivers do not
include performance tickets.

fax or mail it to us as instructed. Sorry, on-line registration isn’t
available at this time.
Fax your
completed, signed form to 305-867-7121. No need for a cover page,
the fax arrives directly in our office. Don’t forget to include your
credit card information for payment. Important: Please call
to confirm that we received your fax and the information is legible.
On occasion, we have received blank pages with no way of knowing who
sent the fax or how to contact them.

ON-SITE
REGISTRATION
Walk-up, on-site registration is welcome and begins at 12:00 p.m.
Sunday, June 20. Participants may register for one or two week sessions
or for individual classes. The one-time registration fee of $20 applies.
Individual master classes are $18 each, $15 for current FDA members,
and do not include admission to performances or other events.
